Pt. sits with hand of test arm resting on thigh. Examiner instructs patient to sit up straight (avoiding slouch posture which could lead to scapular protraction = translation of humeral head anteriorly). Examiner stands/sits slightly behind the pt. and stabilizes shoulder with one hand over clavicle & scapula & with other hand grasps head of humerus with the thumb over the posterior humeral head and fingers over anterior humeral head. Examiner feels for humeral position within the glenoid (typically more anterior). Then gently pushes the humerus anteriorly or posteriorly (most common) in the glenoid to properly seat the humeral head within the fossa. (LOAD). The examiner then pushes the head of the humerus anteriorly noting the amount of translation and end feel (SHIFT)
